Ontario communities, in reaction to the escalating opioid death toll, have established local overdose response plans to address community-specific issues. Public Health Ontario (PHO) leads the COM-CAP (Community Opioid/Overdose Capacity Building) project, which seeks to minimize community-based harms associated with overdoses. This involves collaborating with local communities to pinpoint, develop, and evaluate capacity-building initiatives for effective local overdose prevention planning. Through a participatory design approach, the 'From Design to Action' co-design workshop engaged communities to pinpoint the necessary capacity-building support requirements.
Through the lens of a participatory approach (co-design), opportunities for collaborative dialogue regarding community capacity-building needs arose. During the co-design workshop, three collaborative activities focused on 1) prioritizing scenarios exemplifying challenges within community overdose response planning, 2) ranking the significance of obstacles within each scenario, and 3) prioritizing support systems to address each of those challenges. Fifty-two Ontario-based participants in opioid/overdose-related response plans took part in the study. Participatory materials were constructed based on the insights gleaned from a situational assessment (SA), using data from surveys, interviews, and focus groups. Support priorities and delivery approaches were identified through a voting system which incorporated dot stickers and discussion notes.
The workshop highlighted critical developmental hurdles and paramount support areas, paving the way for effective development and implementation. The following five capacity-building support areas emerged from prioritizing challenges: 1) addressing issues of stigma and equity; 2) fostering trust-based relationships, consensus-building, and continuous communication channels; 3) developing knowledge and maintaining ongoing access to information and data; 4) adapting strategies and plans to structural and contextual changes; and 5) ensuring responsive governance and structural empowerment.
